STEP 1: deadline recommended monday, march 1, 2010
Whether you are a NEW applicant, or a RETURNING applicant for USAVolleyball, this link leads you to our Sun Country Region Instructions Page
to complete your USAVolleyball Membership Application. There are specific instructions, so
please follow them carefully. Parents/Guardians of our club
members must complete this process: FCVBC Representatives cannot
do this, as that is considered fraud under USAVolleyball Guidelines.
APPLICATION AFFILIATIONS:
USAV Region: Sun Country Region
USAV Club: Four Corners Volleyball Club
FORWARD EMAIL your CONFIRMATION to FCVBC Director, Terene Foutz to CoachT4U2@aol.com. FCVBC processes all applications
collectively. DO NOT pay the $40.00 Membership Fee
shown on your email Confirmation: We pay that on your behalf, as
part of your club membership.
STEP 2: deadline recommended monday, march 1, 2010
Complete
the USAV Medical Release Form, requiring BOTH player and
parent/guardian signature. Forward your USAV Medical Release Form to
Jen PItcher, Pagosa Peaks Coach. USAV Coaches must have these Medical Release
Forms physically with them at all USAV-Events., or the team may be subject to USAV Sanctions.
STEP 3: club fees payment deadline, march 8, 2010
Once the roster membership count is established, all FCVBC Club Fees
are due by Monday, March 8, 2010. As per FCVBC Policy, club fees
are non-refundable. Whether or not members participate in individual
events, tournaments, practices, the club fees paid are final.
Costs above and beyond club fees are listed on the "Club Fees Estimates."
FCVBC does not partake of practice content, format, or team travel
organization. FCVBC is serving as an administrative assistance,
as to assure USAV-legal participation for the Pagosa Peaks Volleyball
Club Team. Pagosa Peaks serves as a FCVBC-Affiliate for the 2010
Season. Any fundraising efforts by Pagosa Peaks are SOLELY
through Team Parent Contact and Coach. Any use of FCVBC, Four
Corners Volleyball Club, Inc., name/brand is prohibited, unless
explicitly managed directly by FCVBC Executive Director, Terene Foutz,.
